To Our Patients:
We previously shared with you information regarding our participation in an Accountable Care Organization (“ACO”) which participates in CMS’ Medicare Shared Savings Program. An ACO is a group of doctors, hospitals, and/or other health care providers who work together to provide high quality, patient-centered care to the Medicare beneficiaries they serve.
We want to ensure you have the opportunity to ask us any questions you may have. As a reminder, our participation in the ACO does not limit your choice of health care providers in any way. You still have the right to visit any doctor, hospital, or other provider that accepts Medicare at any time. Your Medicare benefits are not changing.
The enclosed follow-up Notice is for informational purposes only. There is nothing you need to do; however, please feel free to contact us at Family Medical Associate (FMA) at 325-728-2693 with any questions or concerns you may have with our participation in an ACO.
Sincerely,
MISTY LENDERMON, FMA
